This publication is dedicated to the sentinel node (SN) procedure in breast cancer, and more specifically to the treatment of patients in whom isolated tumor cells or micro metastases are detected. The sentinel node procedure is based on the premise that if the first node the breast tissue drains into is clean, the remaining axillary lymph nodes are not likely to be involved and their removal will not be necessary. The introduction of this procedure during the 1990s meant a reduction in overtreatment for many patients, and therefore a reduction in morbidities such as lymph edema and shoulder dysfunction. There have been some concerns, however, which have led to an intensified SN pathology protocol. These concerns, together with an increase in the detection frequency of small nodal metastases and questions about whether the presence of isolated tumor cells or micro metastases is associated with overall breast cancer outcomes, mean that the SN procedure is still the subject of much research and discussion. The results of a number of randomized studies are expected within the next few years. This book is a collection of six published studies related to the prediction of non-SN involvement. These studies show that not only the size of SN involvement but also primary tumor characteristics play a role and that it is of pivotal importance to estimate non-SN involvement for each individual patient. By providing an overview of current evidence, this book will be supportive in making the best decision in current patient care.

Cytoreductive surgery and combination chemotherapy have improved 5 year survival for patients with ovarian cancer, but only 30% are cured due to detection at a late stage and the persistence of dormant, drug resistant cancer cells. Biomarkers could aid in early detection and in personalizing therapy to identify patients who would benefit from conventional or novel targeted drugs. Epithelial ovarian cancers are remarkably heterogeneous, differing in grade, histotype, invasiveness and proliferative index. Low grade Type I serous cancers are associated with mutations of Ras and Raf, but not p53, whereas high grade Type II serous ovarian cancers nearly all have p53 mutations, but rarely mutations of Ras or Raf. An effective screening strategy must detect both types of serous ovarian cancer, as well as the non-serous histotypes. CA125 is expressed by approximately 80% of ovarian cancers, but is elevated in serum at the time of conventional diagnosis in only 50-60% of patients with stage I lesions.

Disease markers have classically encompassed proteins, gene expressions, proteomics and histological abnormalities (dysplasia). Optical properties of the tissue can provide fundamental insights into tissue properties at structural and molecular levels and hence, serve as novel markers of disease. Biophotonics is an emerging field that bridges biomedical optics to medicine. A particularly exciting application of biophotonics is on the diagnosis, detection and screening for cancer. Recently, non-invasive or minimally invasive biophotonics techniques have been developed that sense changes in tissue at molecular, subcellular, microscopic and macroscopic levels. These optical markers are indicative of alterations in tissue morphology, chemical composition as well as functional changes such as changes in microvasculature. In some cases, optical markers have specific histological determinants. In these cases biophotonics techniques may provide a less invasive and fast means of identifying dysplasia (the ‘optical biopsy’). Other groups have focused on using optical techniques to assess the genetic/epigenetic correlates of field carcinogenesis and thus these spectral markers can be used for risk stratification.
